(1) download from our Scholarship Info Page or
(2) call (401) 351-9306 (St. Adalbert's Rectory Providence) and leave your name, address, contact phone number and clearly state your request that a scholarship application be sent to your home or
(4) go to your Rhode Island high school guidance counselor for a copy of the scholarship application and scholarship criteria.
